This service offers individuals the opportunity to live in a family-like environment within a provider’s home. Family Model Providers assist with daily living activities, personal goals, and community integration, ensuring a stable, nurturing environment that feels like home.
Semi-Independent Living services support individuals who live with minimal assistance but still require some structured support to manage day-to-day tasks, participate in the community, and achieve their personal goals.

Supported Living services are designed for individuals who need more comprehensive support to live in their own homes. AGL provides a team of caregivers to assist with daily activities, goal attainment, and community involvement, empowering individuals to live independently within their own residences.
These services focus on helping individuals become active members of their communities. Support includes engaging in social, recreational, and community activities to promote social integration, build relationships, and enhance life satisfaction.
AGL offers wrap-around support to individuals engaged in employment or community programs on an intermittent basis. This includes assistance with transportation, job coaching, and social support to help them succeed in their chosen activities.

Intermittent Employment & Community Integration Wrap-Around Supports
For individuals who may not be able to participate in external community activities, AGL provides homebound support that ensures personal care, goal progression, and social engagement opportunities in a safe, home-based setting.
Supported Employment services focus on helping individuals with disabilities find and maintain meaningful work. This includes career exploration, job placement, on-the-job training, and continuous support to ensure job satisfaction and career growth.

Flexible support services designed to help individuals live safely and independently in their homes or community, tailored to the level of care needed. -

Care provided in a supportive, family-style environment, with different levels of assistance based on the individual’s needs. -
Community Integration Support Services
Assistance in building meaningful relationships, engaging in community activities, and achieving personal goals for inclusion. -
Independent Living Skills Training
Practical training to help individuals develop skills like cooking, budgeting, and managing their daily lives more independently.
